Kunio, a student of Nekketsu High, must fight against several Japanese gangs for beating up his brother.
The player guides Kunio to the battlefield and fights against several gang members. Once you have defeated a certain amount of enemies, the gang leader will come out of the background and help the remaining members beat you up. Once you have defeated the leader, you will see a scene of another gang beating-up Hiroshi (Kunio's brother) and you will proceed to the next battlefield. After all four leaders have been defeated, the game begins again but at a greater difficulty level.

This game was so popular in Japan that several spinoffs and sequels were made in Japan (mostly for the Nintendo Famicom and the Super Famicom). One of these games was Downtown Nekketsu Monogatari which is the Japanese version of the NES cult hit River City Ransom.
"Nekketsu" is a Japanese word that translates into "Hot Blood." It is also the name of the school where the main character studies (Nekketsu Koukou). "Kouha" literally translates to "tough elements," and "Kun" is a suffix that is often used for young men (it doesn't mean "Mr." as some people might tell you).
Kunio was named after Technos Japan's president, Kunio Taki.
This game is the original Japanese version of Renegade. Nekketsu Koukou Dodgeball-bu and its Neo-Geo sequel, Kunio No Nekketsu Dodgeball - Toukyuu Densetsu (both were released in the US as Super Dodge Ball) are spinoffs of this game.
